    Abstract

    The purpose of the thesis is creation of a web application for customized survey regarding different types of respondents, which enables longitudinal research. The application can be integrated in any website (e.g. of a company), where it can be used by guests (as well as employees). The aspect of longitudinal research is reflected in data obtained in a long-term study, which we get by conducting the same survey on the same types of respondents in several intervals. The technologies and tools that I used to create the application are described in the thesis. The application was developed in .NET framework with C# programming language. Analysis and planing of the application are presented which includes the purpose, functional and non-functional requirements as well as data model of the application. The main goal of the thesis is the description of developed application that includes database plan, development of the components and the exact description how the finished application operates.
